In order that they correspond with the amendments to Rules for Cargo Handling Appliances advised in the previously issued Technical Information TEC-0615 of 20 December 2004 and to enable the receipt of applications for the issuance of International Air Pollution Prevention Certificates (MARPOL AnnexVI) for vessels whose completion date is on or after 19 May 2005, the following application forms have been revised as attached;

1. APPLICATION FOR CLASSIFICATION AND SERVICES DURING CONSTRUCTION : Form 1A
2. APPLICATION FOR SURVEYS AND ISSUE OF CERTIFICATES : Form 2A

You are kindly requested to use the new application forms on and after 1 January 2005.
